TEEX has requirements and forms that must be filled out prior to starting clinical rotations.  These forms will be made available after enrollment and must be returned to the Clinical Coordinator at least one month prior to beginning Clinical Rotations.  

 

Requirements Include:

• High School Diploma or equivalent
• Urine Drug Screen
• Criminal Background Check
• Shot Record (Including TB skin test, MMR and Hep. B)
• Proof of Health Insurance (Short Term Ins. is acceptable)
• Copy of Drivers License
